Refer someone for care at home

We accept referrals for Domiciliary Care and Live-In Care from individuals, families, social workers, local authorities and healthcare professionals. Every referral is assessed individually, so we can be sure we’re able to meet the person’s needs safely.

What you can refer to through this page

Both services are delivered in the person’s own home in Northamptonshire.

Aged 18+

Domiciliary Care

Visiting care and support at home — personal care, medication support, meal preparation, mobility assistance, companionship, shopping and appointments. Suitable for older adults, people with reduced mobility or disabilities, and people recovering from illness or a hospital stay.

Adults

Live-In Care

A carer lives in the home, providing a higher level of ongoing support. Suitable for people with long-term conditions, mobility difficulties, or anyone needing more support than visiting care can provide while remaining at home.

About Live-In Care →

Supported Accommodation referrals work differently. Placements for young people are arranged through the relevant local authority rather than through this page. If you’re a local authority professional looking to make a placement, please contact us directly and we’ll put you in touch with the right team. Supported Living referrals are also arranged through commissioning rather than this route.

Who can refer

You don’t need to be a professional

For care at home, families can get in touch directly — you don’t need a social worker to contact us on your behalf.

How it works

From first contact to care starting

We’ll be straight with you about whether we can help. If we’re not the right fit, we’ll say so rather than keep you waiting.

  1. Get in touch

    Call or email with a brief outline of the situation. You don’t need paperwork ready, and you don’t need to know which service you want — we can work that out together.

  2. We respond

    We aim to respond within one business day. Enquiries received outside office hours are picked up on the next working day.

  3. Assessment

    We take time to understand the person’s needs, home environment, routines and what matters to them — and to check that we have appropriately trained staff available to meet those needs safely.

  4. Decision and next steps

    Our management team decides whether we can support the person. If we can, we’ll agree a care plan, funding arrangements and a start date with you. If we can’t, we’ll explain why.

Before you contact us

Useful to have, but not essential

Don’t hold off getting in touch because you don’t have all of this. A phone call is enough to start.

What helps us assess a referral

  • The person’s name, age and address
  • What support they currently have, if any
  • The kind of help they need, and roughly how often
  • Any health conditions or mobility needs
  • Any known risks or safeguarding information
  • How the care is likely to be funded
  • Who else is involved — family, social worker, GP

When we might not be the right provider

We’d rather tell you early than waste your time. We may not be able to help if:

  • The person lives outside our operating area
  • The support needed falls outside our CQC registration
  • The care requires clinical tasks our staff aren’t trained and assessed as competent to carry out
  • We don’t currently have appropriately trained staff available

Support for complex healthcare needs is always subject to individual assessment, our CQC registration, appropriate clinical oversight where required, and confirmation that our staff are trained and competent to meet the person’s specific needs.

Where we operate

Domiciliary Care and Live-In Care are both delivered in Northamptonshire.

We may consider referrals outside this area depending on staffing availability and our ability to safely and effectively meet the person’s needs. It’s worth asking.

Funding

We work with local authorities, commissioners, individuals and families. Depending on circumstances, care may be funded through local authority arrangements, personal budgets or direct payments, private or self-funding, or other eligible health and social care funding arrangements.

Costs depend on the level of support required. Contact us to discuss the person’s circumstances, funding arrangements and the cost of support.
